Please complete the following information.

This form is for Virtual short courses, not for proposing short courses for the Annual Meeting. While courses may be submitted for both, title and content should not be exactly the same, and SMDM will work to schedule an in-person course prior to a virtual course if they feature similar content and the same instructor(s).

Virtual Short Course proposals are reviewed by the SMDM Education Committee. Scheduling of short courses involves many considerations, however if your course is not immediately selected it will be held on our potential course list for the future.
