Would you like to be involved in research at the University of Oklahoma?

I am Dr. Charles R. Ciorba from the School of Music, and I invite you to participate in my research project, which is titled Music Education in the United States: Perceptions from the K-12 Educational Community. This research is being conducted at the University of Oklahoma. You were selected as a possible participant because you are currently employed by a K-12 school district. You must be at least 18 years of age to participate in this study.

Please read this document and contact me to ask any questions that you may have BEFORE agreeing to take part in my research.

The purpose of this research is to determine how members of the K–12 educational community perceive the importance of music education. About 200,000 administrators, music teachers, teachers of other subject areas, and support staff will be asked to take part in this research. If you agree to be in this research, you will be asked to complete a short online survey. Your participation should take no longer than a few minutes. There are no risks and no benefits from being in this research. You will not be reimbursed for your time and participation in this research.

In research reports, there will be no information that will make it possible to identify you. Research records will be stored securely and only approved researchers and the OU Institution Review Board will have access to the records.

In addition, this is an academic not-for-profit research project. Data are collected via SurveyMonkey.com, an online survey system that has its own privacy and security policies for keeping your information confidential. Please note no assurance can be made as to the use of the data you provide for purposes other than this research.

If you do not participate, you will not be penalized or lose benefits or services unrelated to the research. If you decide to participate, you don’t have to answer any question and can stop participating at any time.
