Quality Improvement Collaborative

Thank you for your interest in the Society of Hospital Medicine's Transfusing Wisely Collaborative program.

This survey serves as your application for participation in the program. Applications will be reviewed through September 14, 2018.
 
This application has been developed to confirm that participating sites have the commitment and ability to implement the
program. The Transfusing Wisely Collaborative starts with a two-month introductory phase to help each hospital site get acquainted with the program followed by a 12-month Collaborative Program. The program will formally begin in October 2018.

There is a fee of $8,800 to participate in the program.  Sites that submit their application by August 10th will receive a $200 discount.
