This survey is designed to gather information based on your knowledge and understanding of Disability Services policies and procedures.

information/guidance that enables me to feel comfortable with administering accommodations.

* 6. As a faculty member, I understand the obligations I have regarding confidentiality for students with disabilities.

* 7.  Accommodations for testing such as readers, scribes or the use of adaptive equipment must be permitted for a student whose documentation specifically calls for one of these accommodations.

* 8. The college must provide an accommodation even if it would result in a fundamental alteration of the program.

* 9. I am familiar with the information in the CCRI Student Handbook and/or our website regarding Disability Services for Students (DSS).

* 10. My syllabus contains the ADA accommodations statement as required by CCRI policy.

* 11. I make an announcement on the first day of class about the Office of Disability Services for Students.
