Dear graduates,
Congratulations on your achievement! Please read these instructions carefully.
DEADLINE: Please complete this evaluation by FRIDAY, AUGUST 24. You may enter the survey more than once, but we need your final responses by August 24.
CLE DOCUMENTS: Within six weeks after you submit the evaluation, Michele will email you four documents (course syllabus, staff list, student list, and a PDF version of your graduation certificate) you may need to apply for CLE credits. While we need your name for CLE purposes, your responses will be anonymous when they are shared with TLC's staff and board of directors.
EVALUATION SUBMISSION: To submit your responses, click the "Done" button at the bottom of the evaluation. (You are required to answer all questions, except for question #2. Error messages will be displayed in red font. Errors must be corrected before your responses can be submitted.) When your browser loads the TLC homepage, you will know your responses have been successfully submitted.
