Are you a person living with HIV in Florida?  If so, we want to hear about your housing needs. This is your opportunity to tell us about the housing services you have used or may need in the future.  Your answers will help the Florida Department of Health’s Housing Opportunities Program for Persons with AIDS (HOPWA) program make important decisions about services, funding, and policies.

It should take no more than 15 minutes to complete the survey.  Your responses are completely anonymous.  We have no way to track your responses back to you. Please take the survey only once.

At the completion of the survey, you will have the option of entering the drawing for one of ten (10), $50 gift cards.

This survey will be open from October 14th, 2019 to November 27th, 2019. Please take the survey only once.  Please share the survey with other people living with HIV in Florida so we can hear about their housing needs as well.
