FY26-27 Shape Up SF Capacity Building Mini-Grant Application

The Shape Up SF Coalition (Shape Up SF) formed in 2006 to address growing levels of preventable chronic disease, especially in San Francisco's low-income communities and communities of color.

Mission: To advance health equity in San Francisco by collaborating with community partners on system changes that increase nutrition security and active living.

Vision: Shape Up SF envisions an equitable, thriving community where all who live, work, learn, and play in San Francisco enjoy optimal health.

Shape Up SF’s Capacity Building Mini-Grants are intended to provide funding to build capacity among small nonprofits addressing chronic disease disparities through healthy eating and active living. We define capacity building as whatever is needed to bring a nonprofit to the next level of operational, programmatic, financial, or organizational maturity, so it may more effectively and efficiently advance its mission into the future.

Grants for capacity building typically support infrastructure or administrative expenses, not programmatic expenses. Examples include technology upgrades, staff coaching, accounting help, strategic planning, board coaching, leadership training, etc.

Your organization must meet ALL the criteria below to be eligible:

• Have a focus on healthy eating and/or active living
• Be based in or provide services in San Francisco
• Have an annual operating budget of less than $600,000 for the next fiscal year
• Have board and/or staff leadership that reflects the communities served
• Be a 501(c)(3) nonprofit or have a fiscal sponsor that is a 501(c)(3)
• Be in compliance with California’s charitable registration and reporting requirements

Organizations who received a grant from Shape Up SF in the last 4 years are NOT eligible to apply
