Teens Change the World! Service Program: Pre-Registration Survey

Thank you for your interest in joining our Teens Change the World! Service Program! In order to participate, we're requesting all teens complete this quick survey so we can get to know you a little bit and get your feedback! 

The steps to join the Teens Change the World! Service Program include:
  1. Parent or Teen: Complete our Teens Change the World! Service Program Interest Form
  2. Participating Teen: Complete this survey (only we will see your responses - don't worry!)
  3. Parent or Guardian of Participating Teen: Complete our participation waiver (the link will be shared via email)
  4. Receive a list of upcoming teen service projects and initiatives from our Volunteer Program Manager every few weeks. Sign up for the ones you're interested in! There is no minimum (or maximum) number of projects you can do - do what you can, on your schedule!
  5. At the end of the year, take another quick survey to let us know how things are going and give us ideas on how we can make the program even better!
