LUMA Health Consulting is trying to understand healthcare consumer and consumer advocate perspectives on healthcare access in rural Maryland. By "consumer," we mean anyone who receives healthcare services in the state. By "consumer advocates," we mean family members of patients as well as individuals whose professional role involves patient advocacy.
Survey responses will be used to inform LUMA's assessment of Maryland's Total Cost of Care (TCOC) model. The TCOC Model, launched in 2019, is designed to make healthcare more affordable and keep patients healthy. LUMA's final report will focus on the impact of this model on healthcare facilities, providers, and patients in rural Maryland. You may access this public report in July 2023 through the Maryland Department of Health's State Office of Rural Health's website here.
If you agree to participate, please note your survey responses will not be shared outside of LUMA Health's Consulting team, your responses will not be attributed to you in any way, and you may close out of the survey at any point. Note: Questions with * are required fields.
