The Children's Clinic wants to hear from you! Your feedback is important to us.
We are running a survey so we can learn how to make our clinic better. We use your survey answers to help make changes at our clinic and improve how we take care of your family.
Here is some helpful information:
- You don’t have to fill out the survey if you don’t want to. You can also skip questions you don’t want to answer. This will not negatively affect your care.
- Your answers are anonymous, protected, and kept private. No personal information is shared without your OK and will not negatively influence the care you receive.
- You can win a prize for completing this survey! Here are some of the rules:
- 4 people will be drawn at random to win a prize:
- The drawing will take place on June 16, 2026. If you are the winner, we will call you to ask your prize preference. If we do not hear from you after 1 week, another winner will be drawn at random.
- The winners can choose between a $100 gift card to Amazon or Target, OR you can choose an Amazon Fire HD 8 (2024)
- Only one prize entry is allowed per family
- No child of clinic or Children’s Health Alliance employees can win a prize

The Children's Clinic is a proud member of the Children's Health Alliance and Children's Health Foundation, an association of pediatricians that work together to improve children’s health in Oregon and SW Washington. We are working with the Children’s Health Alliance to send this survey.
