This survey seeks to gather information regarding the novel EMS responses to the opioid crisis nationally. 

We direct this survey to the EMS physicians nationally who are interested in Opioid Use Disorder and the national opioid crisis. Please complete this survey only once.   

We would appreciate any additional comments, elaborations or questions to the survey administrators (listed below) in response to this survey.
