The information provided in this survey will help us identify and address areas for improvement, both in our hospital, and across our local network of hospitals. Questions marked with an asterisk (*) are mandatory for the purpose of collecting data for the Toronto Academic Health Science Network (TAHSN) to measure learner experience across teaching hospitals in Toronto.
