We invite you to take a few minutes to share your feedback by completing this survey, which will help us better understand your experiences in your education and training program. The information provided in this survey will help us identify and address areas for improvement, both in our hospital, and across our local network of hospitals. Questions marked with an asterisk (*) are mandatory for the purpose of collecting data for the Toronto Academic Health Science Network (TAHSN) to measure learner experience across teaching hospitals in Toronto.

To protect your privacy, all data collected will be collated and only shared in aggregate form, both at our hospital, and across our local network, with the intention that you will not be personally identifiable.
