DO NOT USE-Needs Assessment Questionnaire

Please complete the following questionnaire to help the Southeast Addiction Technology Transfer Center (SATTC) understand the needs of its target population (i.e., providers/practitioners who serve people with substance use disorders (SUD), co-occurring mental health conditions, people in need of treatment/recovery and their families and communities) within Region 4. Please submit your responses to this questionnaire by Friday, February 21, 2025.

As a reminder, the objectives of the SATTC during its new grant period (2024 – 2029) include the following:

  • Address workforce retention;
  • Assist in adoption/implementation of evidence-based and culturally adapted behavioral health practices;
  • Assist in the integration of evidence-based trauma-informed practices;
  • Disseminate/support a replicable, evidence-based model for empowering recovery community organizations; and
  • Conduct all activities using an effective accountability infrastructure.

As part of our commitment to behavioral health equity, we are submitting a Disparity Impact Statement to identify and address disparities in project participation and specified outcomes associated with the implementation of project strategies and evidence-based practices across the eight participating states (i.e., Alabama, Florida, Georgia, Kentucky, Mississippi, North Carolina, South Carolina and Tennessee).

Please note:

  1. Your specific responses will be kept confidential and will not be linked in any way to your identity.
  2. A report describing the results of this survey will be developed approximately ONE MONTH after the survey results have been obtained. The SATTC will share and discuss this report with each state’s SSA individually and during its Executive Committee meeting.
