2025 Board Service Nomination Form

Members of the Board of Directors develop the policies for the AFP Chicago Chapter and monitor the execution of those policies. The Board determines its present and future direction including long-range planning, fundraising, governance, financial management, and personnel policy. If you know of a person whom you believe would make a good candidate for the AFP Chicago Board of Directors, please complete the following nomination form.

They will also contact the nominee regarding their interest in board service and review the application process with them. Please note that when contacting the candidate, reference will be given to your nomination.

Nominations for Board service will be taken through July 31, 2024 to allow nominees time to submit their applications by the August 30, 2024 deadline.
