This survey must be completed by each district that received Title III (English Learner) funds during the 2020-21 school year. The questions must be answered based on activities serving ELs during the 2020-21 school year.

Districts must select yes or no to indicate which required and/or authorized activities found in ESSA 3115 were funded with Title III monies during the 2020-21 school year. Districts should select yes only if Title III funds were used for the described activity. If Title III funds were not used for the activity, the district should select no. Please note that some of the activities on the list are required while others are authorized (allowable).

To provide additional clarification of each survey question, the ESSA references are provided in the Consolidated State Performance Report Instruction document.
