Title 1 District - Level Parent Survey 2012-2013
Dear Parent or Guardian: We are in the process of conducting an evaluation for our 2012-2013 parental involvement program and we would like to know your opinion. Please take a few minutes to complete this survey. If you have a child at more than one Title I school, please consider filling out multiple surveys. The responses will be used to improve this year's parental involvement program.
