Talk to Me - Registration and Participant Information Consent Form
Project Information and Participant Consent
You are invited to take part in the evaluation of the SVHM Talk to Me Application which aims to improve communication of care needs for people from Culturally and Linguistically Diverse (CaLD) backgrounds. We are interested in your feedback and opinion about the use of this technology to help manage basic, non-clinical healthcare conversations. The consultation is through a survey and questions may include providing your opinion on how easy the app was to use, how well it is designed, whether you think it is suitable to be used in a healthcare setting, and how satisfied you were with it overall. You may also be asked explain what you liked most and least about the app.
When you register below, you will be asked to provide some background information about yourself and your experience using translation technology as a person living in the community from a CaLD background. After filling in this registration form and providing your consent to participate, you will be sent the link to the app so that you can explore the tool as well as the feedback survey to summarise your experience.
There are no costs associated with participating in this consultation. You will need to have access to either an iPhone, iPad or desktop computer with wi-fi connection in order to participate.
Upon completing your evaluation of the app, you will receive a gift card to the value of $30 for your time.
Participation in this consultation is voluntary. If you decide to take part and later change your mind, you are free to withdraw from the project at any stage. Your decision whether to take part or not to take part, or to take part and then withdraw, will not affect your current or future relationship with the City of Melbourne or St Vincent's Hospital Melbourne.
At the end of the consultation, a report of the research findings will be written for St Vincent’s Hospital Melbourne and the City of Melbourne. At your request, the researchers will also provide you with a one page summary of the project after the project ends which is in the end of 2022.
By signing the consent question 6-10 below, you consent to the principal investigator collecting and using personal information about you for the consultation. No information that can identify you will be collected in this project. All information collected will be held at St Vincent’s Hospital Melbourne in password protected electronic files.
