Honoring Our Local Heroes | 2024 Application
Swampscott Veterans’ Services is proud to announce our campaign for "Swampscott Hometown Heroes" banners. Banners will proudly be displayed in the Town to honor local residents who have served or are currently serving in the Armed Forces.
We are seeking to honor local Veterans with Hometown Heroes Banners. Each Veteran will have enlisted in the military from Swampscott. We plan to unveil 15 banners for Phase 1 of “Swampscott Hometown Heroes” for Veterans Day 2024. Banners will fly proudly in Swampscott and when they are retired they will be presented to the family.
Registration is open from through September 13th. Please have all the information below available as we can only accept completed applications.
There are only two requirements to be eligible:
1) the Veteran must be a resident of Swampscott when they enlisted in the military
2) the Veteran/Service Member must be currently serving in the United States Armed Forces as a satisfactory participant or have been honorably discharged. A DD-214 or statement of service will be required for verification by the Town and Swampscott Veterans' Services.
The form below will request the following information:
1. Requestor's Name (must be the Veteran or an immediate family member)
2. Veteran's First Name, Last Name, Branch of Service, Rank, Era of Service and Location of Service
3. Phone Number and email for a representative of the Veteran for future communication
4. High Resolution photograph in uniform (pdf or jpg only)
Completed applications for Phase 1 will be accepted on a first-come, first-serve basis.
