Introduction to the V.O.I.C.E.S. Survey

The mission of the Surry County Substance Abuse Recovery Office is to build a full “Continuum of Care” that eliminates impediments for Surry County residents seeking substance use treatment and recovery services.

PURPOSE
The purpose of this survey is to: help evaluate medication storage patterns, community perspectives regarding substance use, knowledge of current service availability and community needs.

INTENDED SURVEY AUDIENCE
This survey is intended for the audience of citizens over the age of 18 and living in Surry County, NC.

HOW LONG WILL IT TAKE ME TO COMPLETE?
It is expected to take most users about eight (8) minutes to complete.

STATEMENT OF CONFIDENTIALITY
We want to assure you that your responses to anonymous surveys are completely anonymous. Responses to anonymous surveys cannot be traced back to the respondent. No personally identifiable information is captured unless you voluntarily offer personal or contact information in any of the comment fields. Additionally, your responses are combined with those of many others and summarized in a report to further protect your anonymity.

USE OF DATA
Summarized data collected in the survey will be presented in a formal and published reports of the Surry County Substance Abuse Recovery Office. Results will also be available on the Surry County Substance Abuse Recovery Office Website (www.surrycountycares.com) by clicking on the Data Menu.
