Dear Summit Lacrosse Club Parents,
We hope that you and your child, or in many cases, children, enjoyed a great season this past spring. As you may know, the mission of the Summit Lacrosse Club is to teach the boys and girls of Summit the game of lacrosse within an environment that is, in equal measures, both competitive and enjoyable. Our goal is to have fun and improve all players as much as possible.
In an effort to continuously improve our program, we request that you provide feedback and suggestions as to the quality of the experience that both players and parents had this past season.
Please note that this particular survey is only being sent to parents of children in grades 2 and above.
Unless you provide us with your contact information at the end of the survey, all submissions are anonymous. We appreciate your candid feedback and continued support of the program.
