I accept enrollment in the 2023 summer externship course.  By enrolling, I accept professional responsibility for working at my externship site 135 hours during the designated 4-6 week period between May 15– July 28, and abiding by confidentiality and other ethical obligations.  After I complete this acceptance form, withdrawal from the program is not permitted.

Further, I understand that I must complete academic coursework during the period including submitting a learning goals statement, a writing sample, two reflection papers, weekly hours submissions, a final evaluation, and that my Tulane faculty supervisor may require I check in with them periodically by telephone or other means of remote communication. I also understand I am required to attend a mandatory summer externship orientation on April 24, 12:00 - 1:00PM.
