Instructions to the Survey Respondents

Thank you for agreeing to participate in this survey on the use of evidence-based practices (EBP) by practitioners in community rehabilitation agencies to deliver services to people with disabilities.

The survey should take about 20 minutes and you can return to it as long as your web-browser is open and as your time permits. The survey will ask about you and your organization and then asks you to rate four sets of statements about your:

(a) confidence in applying EBP,
(b) current use of EBPs,
(c) supports and barriers available to you, and
(d) expectations and potential value of EBPs in your work.


The survey questions are not considered to be harmful in any way, and it is highly unlikely that responding to this survey will cause any discomfort or distress. By responding to survey questions, there is a risk that you could reveal personal, sensitive, or identifiable information about yourself or others within your responses. There is also a risk of becoming identifiable based on your responses to some of the questions (e.g., combination of the license(s) you have, your job title and state your agency is in could make you identifiable). Because of this, there is the possibility of a breach of confidentiality. However, we are not requiring identifiable information from you, and any personal, sensitive or directly identifiable information about you, if revealed, will not be included in any publications. Also, any personal, sensitive or identifiable information about others, if revealed, will not be included in any publications. Only the study team will have access to survey data and only aggregated data will published; not individual data/responses. You can also choose to skip any survey questions at any time.

Although there are no direct benefits, information from this study will help us to generate research and training materials to support evidence-based practices. These practices may enable you to provide more effective services within VR delivery systems, resulting in improved employment outcomes for people with disabilities.

By completing this survey, you are giving your "informed consent," but you can select to skip any questions that you do not wish to answer prior to submitting. You can also choose to quit the survey any time before submitting your responses without penalty and your responses will not be included in our data.
