A comprehensive discharge resource booklet was developed  to support clinicians to make appropriate referrals for ongoing care in 2015. Details of external services/organisations, their addresses and inclusion criteria were included. The discharge resource booklet is now only three years old so there is no need to update the information right now as it is fairly up to date.
