We are no longer using this version of the Exit Interview Questionnaire. The newer version of the Exit Interview Questionnaire is live in our Human Resource Management System, Workday(R), and collecting Exit Interview Questionnaire responses. Upon voluntarily separating from Oregon state government, the Exit Interview Questionnaire is automatically sent to the separating employee via that individual's Workday account.
