The purpose of this form is to collect information to assist Spray Marks to determine the outcome of your application. This information will be held by Management and will be accessible only to management staff. If your application is unsuccessful, then any CV submitted will be shredded, unless otherwise requested.
You will need to attach:

- A copy of your passport (if not a NZ resident)
