The materials in this survey are for the Site Coordinator (main organizer of the series):
• Site Coordinator’s Evaluation – Please think about these questions throughout the series and answer them after the series is over. We depend on your thoughtful comments to help NBNR improve.
• In-Kind Tally for the Series – It is important to keep track of time, expenses, and materials used for the program. Please see the top of the In-Kind form for an explanation. The Contribution Log, provided to help you gather information for the In-Kind Tally, is available for download on the MHC website if needed.
• Audience Report – Please give us a count of the total number of people present at each session of your series. This information is required for our reports and funding. This form is at the end of this evaluation, but you may also mail it to us if that is more convenient for you.
If you attend the NBNR sessions, please also fill out the Tutor/Instructor/Staff Evaluation, which is available separately on the MHC website.
