Site Administrator/Proctor Testing Agreement

If you select Site Administrator below, this means that you:

• are our single point of contact
• confirm that you are not an instructor
• oversee test administrations
• can proctor exams or must ensure all proctor(s) have completed this form
• will only share login information with proctors that have completed this form
• will be responsible for distributing results/certificates

If you select Proctor below, this means that you:

• assist the Site Administrator with proctoring exams
• cannot be an instructor in the subject area covered by the exam
• understand all communications will only be with the Site Administrator
• understand the CTTC will not share login information with you

If an extenuating circumstance happens where the Site Administrator cannot perform the duties outlined above, you must contact our office immediately to discuss a resolution.

As the Site Administrator/Proctor, I:

• Agree to verify with the instructor that students have completed coursework prior to testing.
• Understand to keep all test materials and access information in a secure, locked location.
• Agree to coordinate/conduct test administration within a controlled environment with oversight by approved proctors.
• Agree to retrieve/distribute results and certificates to all candidates in a confidential and secure manner.
• Understand that all materials used for certification testing are proprietary and confidential.
• Agree to maintain confidentiality of all testing materials including protecting all items from loss, unauthorized access, and reproduction.
• Understand that Instructors cannot be Site Administrators/Proctors or have access to any certification assessments.
