Informed Consent Statement

You are being given the opportunity to volunteer to participate in a project conducted through John Brown University and the city of Siloam Springs.


1. The purpose of this survey is to gather feedback regarding citizen satisfaction of the local government.

2. As a citizen of Siloam Springs, you have been chosen to participate in this study and provide helpful feedback to improve local government services.

3. Participants will answer a series of questions throughout this survey. The topic of the questions involve quality of life and characteristics of Siloam Springs, satisfaction with local government services, and various demographic questions

4. There are no known risks in this study.

6. The survey results will be anonymous and none of the data you provide will ever be linked to your identity. None of the following questions are required. Participants are free to skip any question(s) as they wish.

7. The data received from this survey will be stored in SurveyMonkey. It will be protected by a password. Only the researchers will have access to the data. The researchers will not have any way to connect any data to anyone’s identity

8.  Persons under the age of 18 should not participate in this study.

9. Refusal to participate in this study will have no effect on any future services you may be entitled to from the city. You are free to withdraw from the study at any time without penalty.
