1. Show Me YOU: An LGBT Health and Wellness Questionnaire

Thank you for your participation. This questionnaire hopes to evaluate the health and well-being of lesbian, gay, bisexual, transgender (LGBT), and other sexual and gender minorities in the state of Missouri. This questionnaire will use the phrase sexual and gender minorities to describe any individual whose sexual or gender identity, orientation or practice differ from the dominant culture. While clinical, this phrase is used with the intention of being inclusive of both identity and behavior.  Conducted by PROMO, Missouri’s Statewide LGBT Advocacy organization, through funding by Missouri Foundation for Health, Wells Fargo, and the William Kerr Foundation we hope to assess existing and lacking resources in an effort to enhance the overall health of Missouri’s LGBT population. 

 This survey will take you approximately 10 minutes to complete. Your feedback is important to us.  The survey is anonymous and confidential.  If you would like the results mailed to you or to stay involved with PROMO, there is a space at the end of the questionnaire to include your information. Your answers will be only accessible to the researchers of the study.      

You have the right to quit the survey at any time, and you can skip questions that you do not feel comfortable answering. Please do NOT take this survey if you are under the age of 18. 

If you are taking the online survey, we recommend you delete any temporary files or cookies after you have completed the survey. Once the study is completed, we would be happy to share the results with you if you desire.
