Participant Information Sheet

‘Sex Ed in NSW’ is a confidential survey conducted by Family Planning NSW, in partnership with the Ministry of Health.

We want to find out the experiences, need and opinions of young people in years 8 to 12 across NSW schools in relation to sexual health education that is provided part of the PDHPE, Crossroads and student Wellbeing curriculum.

This information will be used to guide the development of resources, professional education and programs to support state-wide delivery of sexual health education.

This survey does NOT ask about your own (or anyone else’s) sexual behaviour or sexual experiences.

The survey will take about 20 minutes to complete. Participation is completely voluntary.

You are eligible to participate in the survey if you:

·         Live in New South Wales; AND

·         Are currently enrolled in years 8,9,10, 11 or 12

What do I need to do?

After reading this form, you will be asked to give your consent to participate by ticking the “YES” button at the bottom of the survey homepage. Please read carefully the information in this form before making your decision.

If you agree to participate and tick the “YES” button, you will be routed to the survey. Please follow the prompts.


After completing the survey, you will also have the opportunity to enter into a draw to win one of ten $50 gift cards (iTunes, Westfield, or phone credit). Your contact details will not be linked to your survey answers, as the system will collect them separately.

You can freely discuss this survey with your parent/guardian or even seek their assistance to complete this if you wish.

What if you don’t want to take part in the project?
That is OK, it is not compulsory. Taking part in this study is completely voluntary, so you can choose to participate or not. You may also withdraw your participation at any time without having to provide a reason. It will also not affect your relationship with the staff at Family Planning NSW or anyone else involved in the study now or in the future.

Can you be identified as participants?
No, you will not be identifiable. Your individual answers will not be shared with anyone except the researchers, and no information will be reported in any way that will identify you or anyone else.

To protect your privacy, data will be automatically de-identified by the survey system as you complete the survey. This means that if you provide an email address, for instance to enter the prize draw, your email address will not be linked to your survey responses.

In any reports or articles arising from this study, any information included will not be able to be linked back to you.

How will the information be used?
What we find out from the survey will help us develop new resources and training for teachers across NSW for sexual health education of young people We may also share some of the information with other health organisations and the public, but none of this will identify you as an individual.

What risks are involved if you participate?
We do not anticipate any risks associated with your participation in this project. However, if anything makes you feel upset or uncomfortable, you can stop taking part at any time.
