DeKalb Township's 2026 Wise & Well Senior Summit Registration Form Survey

Please use this form to register for the 2026 Wise & Well Senior Summit, to be held at the DeKalb County Farm Bureau on August 6, 2026, from 8:30 AM - 3:30 PM.

Lunch will be provided for guests who pre-register by July 29, 2026. Registration may close before this date if event capacity is reached.

Parking will be available on-site as well as in nearby parking lots with free shuttle service to the event.
