Living Healthy Workshop Planning Survey (Seneca County)

Your input is needed to help us plan and offer Chronic Disease Self-Management Programs (CDSMP) in Seneca County. The CDSMP is an effective self-management program for people with chronic diseases and/or caregivers of people with chronic diseases. Managing a chronic disease can be complex and frustrating. CDSMP workshops provide participants with tools and strategies for managing and coping with chronic health conditions including: techniques to deal with pain, fatigue and physical limitations, appropriate exercise and medication use, effective communication and proper nutrition.
