* 1.    I have chosen to participate in the Self-Help Center.  I understand that I will not be provided an attorney or legal advice, but that I will be provided standard forms for my own use and may be directed to other possible resources for assistance in my case.
     I fully understand that it is my responsibility to determine whether the forms I receive are complete and/or proper, to complete any forms I choose to use, to properly file and serve papers, to schedule and attend hearings and conferences, and to find and present evidence.
     I realize that I alone am responsible for any outcome in my case which may be caused by my error or inexperience, that an attorney could help me to understand and to present my case, and that the outcome may not later be changed solely because I chose to represent myself.
     Knowing these things, I have decided to proceed without an attorney.  I have received, read, and do understand these instructions as to the limited role of the Self-Help Center.
