The goal of this survey is to see if parents, students and staff are aware of the School Based Health Center here on campus. If you are aware of the School Based Health Center, we would like you to identify barriers on why a student would NOT use the School Based Health Center. To utilize the School Based Health Center, consents are available in English and Spanish, and can be found in the administration office, or our school website. A signed consent is kept on file the whole time a student is enrolled at our school. We appreciate your time and feedback as the School District Manatee County hopes to bring more School Based Health Centers to our schools. * 2. Did you know that the School District Manatee County has a partnership with MCR Health and students have access to a School Based Health Center right here on campus, Monday - Friday from 7:30AM - 3:30PM, and can provide physicals, immunizations, labs/tests, sick visits, etc.?

* 3. Did you know that MCR Health accepts most private insurance, Medicaid, and has sliding fee scales for those with no insurance?
