Dear LHS Board Members: this anonymous survey is designed to understand your experiences, perceptions and opinions about the current role of the board of directors. The results will help determine how the board can support LHS to achieve its mission and achieve self-sufficiency.

Please complete the survey by October 15th. A report of the findings will be reviewed by the strategic planning committee and then distributed to the entire board before the strategic plan retreat to be scheduled soon.
