This form is for reporting any concern, incident, or allegation relating to the safety and wellbeing of anyone connected to TAFISA's activities, whether or not limited to a Board member, trainer, staff member, volunteer, or delegate, and including participants at community events. It is based on
TAFISA's Safeguarding Policy.
Reports and information will be shared only where necessary to assess and respond to the concern, subject to applicable legal and safeguarding obligations. Reports can also be made anonymously; however, without contact details, updates on the investigation might not be possible.
If you believe someone is in immediate danger, please contact local emergency services first; this form is not a substitute for urgent action. The form takes approximately 10–15 minutes to complete.
