Defined as:  Create a culture of performance and data-driven quality improvement.  Specifically, social service leaders implement a system of accountability and quality improvement at all levels based on agreed upon performance benchmarks, reporting, improvement strategies, and data sharing in collaboration with health care decision makers.

Defined as:  Advance equitable, shared financing of social care and shift to risk-based payment overtime. Specifically, finance services that address social determinants through multi-payer arrangements that build community capacity. Transition from fee-for-service payment to value-based/risk-based payment models for delivering social services in comprehensive, coordinated care environments.
