2.How would you rate your overall experience with the Summer Youth Employment Program?(Required.)

Excellent! It was better than I expected and was a great experience.

Good. It was a good experience and it was helpful to all involved parties.

Average. My experience was no better or worse than I anticipated.

Bad. My experience was not at all what I anticipated. I found no benefits to participating.

3.How would you rate the overall experience with your youth employees?(Required.)

Excellent. The youth employees were excellent and exceeded my expectations. They were well prepared, eager to learn and I would hire them again or I plan to keep/hire.

Very good. The youth employees were a great asset to my company and had great work ethic.

Average. The youth employees were reliable and positive but could have been more eager and prepared for working.

Below Average. The youth employees were not meeting our expectations and were not bringing positive benefits to my company.

4.How would you rate your overall experience with the Community Based Organization you worked with?(Required.)

Excellent. They created an excellent working relationship and handled all of my needs. I would work with them in the future.

Very Good. They were very involved with the youth's work and performance and were easy to contact and work with.

Average. They worked with us to get the youth placed but were not very involved past initial placement.

Below Average. They were not easy to work with or contact and did not seem concerned with the placement and work performance of the youth in their program.

5.Did the Community Based Organization provide any of the following to help you with youth placement and transition?(Required.)

Technical Assistance (assisted you in all aspects of the program including paperwork and placement information).

Onsite visit to your location to view location and work experience progress.

Ongoing communication opportunities (kept in contact with your business throughout the program and were easy to contact if you had questions).
