* 1. This is the parental consent form for the 2019 British Youth Championships (BYC)  finals, and as such should be completed by the PARENT/GUARDIAN of all fencers who are competing at the event.

PLEASE NOTE WE WILL NEED ONE FORM PER CHILD IF YOU HAVE MULTIPLE CHILDREN COMPETING AT THE EVENT. YOU CAN COMPLETE THE SURVEY AS MANY TIMES AS NECESSARY FOR EACH OF YOUR CHILDREN BY FOLLOWING THE SAME LINK.

* 9. Having qualified for the BYC Finals I hereby consent to my son/daughter taking part in the British Youth Championships Finals on 4th -6th May 2019.  

I accept that neither the Organisers nor British Fencing can be held responsible for any accident, loss or damage
sustained at the Championships.

I understand that all persons wishing to take photographs or videos at these events must register their name, address and telephone number with the organiser at the event. I must also notify the organisers of the event if myself or my child wishes to withhold their permission to be photographed in close-up, but realise that it is impracticable to prevent general photographs from including my child.

I wil ensure that my son/daughter is familiar with the British Fencing code of conduct which can be found at
