All students will receive a digital certificate upon program completion. Completed certificates will also be added to your official University of Utah transcripts. 

You should receive your digital certificate within 5 business days from when you submit this form. Once you "claim" your digital certificate/badge, you will be able to share your accomplishment on social media sites such as LinkedIn and Facebook. When clicked, each digital certificate shows the requirements behind the credential, the issuer of the certificate, and information about the knowledge and skills you have gained by completing the certificate.

Please note that completion of this form requires a copy of your unofficial transcripts with grades posted for all courses.
