Short fast survey on specialist services and Health Care Homes

The purpose of this survey is for the College to have an evidence based understanding of how the Health Care Homes trial (Commonwealth Government led) in Primary Health Networks throughout Australia  have involved and impacted specialist services and their adult patients.
The data are not being published and all responses are de-identified. The data  informs the College on the impact the HCH trial is having on  Fellows, and observed  impact on patient outcomes. This direct source of specialist experience is relevant to the College's engagement in health reform, particularly discussions on possible extensions of the HCH trial to include specialist care.

This survey has ethics approval (CH62/6/2018-086) from the Human Research Ethics Committee, Concord Repatriation General Hospital.
