1. Medicines Optimisation Strategy Questionnaire

South West London and St George's Mental Health NHS Trust are developing the next medicines optimisation strategy.
The use of medicines is the most common healthcare intervention in the NHS. Medicines optimisation is a person-centred approach to safe and effective medicines use, to ensure people obtain the best possible outcomes from their medicines.
We want to work with our stakeholders across South West London (Sutton, Merton, Richmond, Kingston and Wandsworth) to shape the new strategy. We are interested in hearing and including the views of people who use our services, carers, work with us, GPs, commissioners and community groups.

It would really help if you could complete this short survey to share your thoughts, views and ideas. The survey should not take longer than 5 minutes to complete.

Thank you for taking the time to complete this survey
