St. Joseph's Health Care London is leading the development and implementation of a strategy to improve access and delivery of care for frail seniors, across the South West Local Health Integration Network (LHIN) region.  Providers, seniors, and family caregivers are being asked to provide their input into the development of a vision for the South West Frail Senior Strategy.  By completing this short (5 minute) survey, you will help us better understand the current state of seniors care in your area, and identify opportunities to increase integration and access, and to improve seniors' health outcomes and care experiences.
