Accessibility Questionnaire - Parents/Guardians, Community Members, Service Providers

Accessibility is the state of having programs, services, and environments that allow all individuals to participate fully in society without encountering barriers. 

This questionnaire aims to gather information regarding accessibility issues or challenges that our parents/guardians, volunteers, community members, and service providers may be experiencing in SD10. The information provided in this survey will be used to inform the development of priorities to advance accessibility in SD10.
