CONSENT TO COMPLETE THE SURVEY
On behalf of your organisation you have been asked to fill in this online survey. It should take no more than 30 minutes of your time.
Before you consent, on behalf of your organisation, to participate in this study it is important for you to understand why the research is being done and what it will involve. The purpose of this study is to: investigate the level of provision of HCV-related services in drug consumption rooms; and, explore the association between provision of these services and general characteristics of DCRs, their clients and the services they provide.
The participation of your organisation in this study is voluntary.
It is auspiced by the Sydney Medically Supervised Injecting Centre, the Rogenboog Groep and the International Network of DCRs & has ethics approval from the Human Research Ethics Committee of SESLHD (Australia).

The decision not to participate or to withdraw will not impact your relationship with the Rogenboog Groep, the International Network of Drug Consumption rooms or the Sydney Medically Supervised Injection Centre.
Other information about the study:
- Participation in this study will not cost anything. You will not be paid for participation.
- The study is funded by the Sydney Medically Supervised Injecting Centre.
- The only foreseeable risk in taking part in this study is that your organisation, if it is the only DCR in your country, may be identifiable. If this is the case, at the end of the online form, you can indicated that you do not give permission for your results to be presented in any way that will identify your organisation.
- The information from this research study aims to: (1) benefit others who are looking into expanding drug consumption rooms worldwide; and, (2) increase access to HCV-related prevention and treatment among vulnerable drug using populations; however there might be no direct benefit from the study to you.
- Any identifiable information, such as your name and the name of your DCR, that is collected in connection with this study will remain confidential.
- We will ensure that the identification of your organisation will only be used for our internal check against duplicate information per organisation; that your name will only be used in case you need technical assistance with filling in the survey; and that your email address will only be used to send you the results of the research, if you indicate (below) that you wish to recieve them. Your details which will be held securely at the Sydney Medically Supervised Injecting Centre.
- We plan to discuss/publish the results in a survey report and in a peer-reviewed journal. The results will also be made available via http://www.drugconsumptionroom-international.org/. In any publication, information will be provided in such a way that you and your organization cannot be identified.
