Welcome to the DAIP Survey

Thank you for participating in our survey. Your feedback is important.

It is a requirement of the Disability Services Act 1993 (the Act) that public authorities develop and implement a DAIP that outlines the ways in which the organisation will make access to its information, services and facilities equal for people with disability, their carers and families. The Act identifies specific outcomes to be achieved by organisations in providing services to clients with disability.

The Shire of East Pilbara is reviewing its current DAIP, and invites you to complete this survey to provide feedback.

If you require assistance to complete the survey please contact the Coordinator Community Wellbeing on 9175 8000.

If you would like to view the current DAIP an electronic copy is available from the Shire of East Pilbara website; or printed copies can be posted to you on request by contacting the Coordinator Community Wellbeing on 9175 8000. Copies are also available at all Shire libraries
