Middle Meningeal Artery Embolization for the treatment of Chronic/Acute on Chronic Subdural Hematoma

For the purpose of this survey, a subdural hematoma is defined as a chronic/acute on chronic subdural hematoma, NOT an acute subdural hematoma, that may cause mild neurologic deficits or be asymptomatic.
