Event Questionnaire

Please fill out this survey so we can adequately assess how to best serve you in your rental and check our availability. If you need a more immediate response or would like to talk to us, after your survey is complete, please call or text Charissa at 267-315-7250. Note, even if you call, we will ask you to fill out a survey unless you have already done so.
1.Please agree to these terms before continuing: This rental is for a private parties only. Events should be structured in style (ie. receptions/baby showers/quinceañeras), no 'club style' parties. There is no cover charge allowed. No Alcohol is permitted on the premises. It is also a drug-free, tobacco-free, alcohol free, weapons free zone inside and outside the building and parking lot across the street. No vulgar language. The SRYC is open every Friday and Saturday during the school year for grades 6-12. If it is open during the time of the rental, only youth in the appropriate grades may be allowed to use the youth center.(Required.)
