INTERNET-BASED INFORMED CONSENT TO PARTICIPATE IN A RESEARCH STUDY

Understanding Science and Math Students’ and Professors’ Perceptions of a Teaching Career

You are being asked to participate in a research study because you are enrolled in a science or math course at Queens College this semester or have declared a science or math major.
 
Purpose: The purpose of this research study is to investigate science and math undergraduate students’ perceptions of the teaching profession.
 
Procedures: If you volunteer to participate in this research study, I will ask you to complete a questionnaire about how you perceive the teaching profession.
 
Time Commitment: The questionnaire takes approximately 10 minutes to complete.
 
Potential Risks or Discomforts: While it is unlikely that you should experience any discomfort reflecting on your perceptions of the teaching profession, you may refuse to answer any question you wish and still remain in the study. You may also end your participation at any time.
 
Potential Benefits: You may benefit from reflecting on a teaching career, learning about local teacher salaries and benefits, and learning how to further explore a teaching career via information shared with you following survey completion. This study will add to institutional knowledge about science and math students’ career interests, which may help the College support future students.
 
Payment for Participation: You will not receive any payment for participating in this research study.
 
Confidentiality: The information you provide in the questionnaire will not be personally identifiable. Data will be saved on a password protected computer. Publications, reports, and/or presentations that result from this project will not identify anyone by name. The research team, authorized CUNY staff, and government agencies that oversee this type of research may have access to research data and records in order to monitor the research. I will make my best effort to maintain confidentiality of any information that is collected during this research study. I will disclose this information only with your permission or as required by law.
 
Participants’ Rights: Your participation in this research study is entirely voluntary. If you decide not to participate, there will be no penalty to you, and you will not lose any benefits to which you are otherwise entitled. Your participation or non-participation in this study will in no way affect your grades, your academic standing with CUNY, or any other status in the College. You can decide to withdraw your consent and stop participating in the research at any time, without any penalty.
