You were selected as a nominee of interest for the 2016 STEM Education Awards, in the category of “STEM Day.” This category was created to recognize the best activities conducted by a school or extracurricular program during Georgia STEM Day.

The following questionnaire has been created to help the judges assess nominees in order to determine finalists in each category. Please submit your completed questionnaire no later than close of business on Monday, September 12th to continue in the nomination process.
